AOD COunselor/ CAse Manager

Duties include, but are not limited to, conducting assessments and screenings to determine appropriate treatment and services, maintaining a directory of resources, working to meet patient needs effectively overcoming individual barriers to care, providing supportive individual rehabilitation services, organizing and facilitating education groups, and doing outreach for the AOD program.

EHR Support Specialist

Long Valley Health Center is seeking an EHR specialist to join our team. Our ideal candidate would have eClinical Works (ECW) experience. The EHR specialist works to implement and customize EHR enhancements as well as training and supporting best practice workflows. Sample duties include providing general user EMR support in person and/or remote, providing initial and ongoing EHR training for providers and staff, participating in planning for new EHR modules and roll out of new features and builds, and managing support issues with HITCare, eClinicalWorks and/or other clinical IT vendors.

Patient Service Representative  Medical Team

Long Valley Health Center is seeking a Patient Service Representative to join our busy office. We are looking for someone who has excellent communication skills and can provide service in a compassionate, professional, and friendly manner to our patients while working in a fast-paced environment. Are you someone with a knack for finding solutions? Passionate about quality customer service? Is teamwork part of your DNA? If you're a can-do candidate with plenty of positivity to boot, this is the job for you! As a Medical Receptionist, some sample responsibilities are screening patients at the door, answering phones and greeting incoming patients and visitors, performing patient registration and insurance verifications, collecting co-pays, and coordinating with other teams for timely care of patients.

Driver

LVHC is seeking a driver to join our team providing safe, reliable, and courteous non-emergency medical transportation to our patients! Our transport vehicle is wheelchair accessible. We would prefer someone who has knowledge and experience driving on Laytonville area roads. Duties include transporting clients to appointments in a timely and safe manner, assisting clients to and from the vehicle, properly securing wheelchairs inside the vehicle, creating a welcoming environment for clients at all time, ensuring proper sanitation and disinfecting of vehicles after each passenger, wearing proper PPE at all times while providing safe transportation service, and maintaining service needs for the vehicle. Must be 18 years of age. A California driver's license with a clean driving record is required. No previous experience is needed.
