The Long Valley Health Center is dedicated to providing you with the highest quality, cost effective medical care. We accept all methods of payment and allow patients to make monthly payment plans.

INSURANCE & PAYMENTS

Payment Methods: 

 

We accept cash, money orders, checks, and all major credit cards (including Care Credit).

 

Payments can be made at the front desk, mailed in (P.O. Box 870, Laytonville, CA 95454), or called in to (707) 984-6131 x293

 

We are happy to work with patients to develop monthly payment plans for anyone who is unable to make immediate full payments.

 

Accounts with outstanding balances that have not had any payments within 90 days will be in jeopardy of going to Collections.

 

 

Insurance: 

 

Patients with insurance who receive services at Long Valley Health Center facilities are asked to make their copayment on the day of service, any remaining charges (such as deductibles) will be billed to you at a later date once we have received a payment from the insurance company.  We generally accept the following insurance carriers, but it is important to confirm directly with your carrier what LVHC services your plan covers:

 

  • MediCal/Partnership (call 800-863-4120 to check that you are assigned to us)

  • MediCare

  • Most major insurance providers

-Check with your insurance provider directly prior to your appointment to verify coverage

-We do not accept any HMO insurance plans

 

Sliding Fee Discount Program for the Uninsured: 

 

The sliding fee discount program includes a schedule of discounts for services to ensure that financial barriers to care are minimized for patients who meet certain eligibility criteria that is based on family size and income.

Special Financial Support Services: 

 

There are a number of Programs that offer financial assistance and/or temporary coverage for those seeking specific health care services*.  Some of these programs include:

 

Child Health Disability Prevention Program:

The CHDP program oversees the screening and follow-up components of the federally mandated Early and Periodic Screening, Diagnosis, and Treatment (EPSDT) program for Medi-Cal eligible children and youth.  The CHDP Gateway, establishes presumptive eligibility programs for children/youth which serves as the entry point for children to enroll in ongoing health care coverage through Medi-Cal or the Healthy Families program.

 

Every Woman Counts:

Every Woman Counts (EWC) provides free clinical breast exams, mammograms, pelvic exams, and Pap tests to California’s underserved women.  Every Woman counts provides retroactive coverage for eligible services received up to seven days prior to enrollment.

 

Family PACT:

Family Planning, Access, Care, Treatment (PACT) provides comprehensive family planning services to women and men including all FDA approved forms of contraception, emergency contraception, pregnancy testing with counseling, preconception counseling, male and female sterilization, limited infertility services, sexually transmitted infection testing and treatment, cancer screening, and HIV screening. Individual client reproductive health education and counseling is an ongoing component of all services.  Family PACT provides retroactive coverage for eligible services received up to seven days prior to enrollment.

 

Presumptive Eligibility for Pregnant Women:

The PE for pregnant women program allows the Long Valley Health Center to grant immediate, temporary Medi-Cal coverage for ambulatory prenatal care and prescription drugs for conditions related to pregnancy to low-income, pregnant patients, pending their formal Medi-Cal application. Presumptive Eligibility provides retroactive coverage for eligible services received up to seven days prior to enrollment.

 

*Patients will be charged for any services they receive beyond what special programs cover even if the patient receives these services during an appointment that is covered by a special program.

Any such claims must be filed in Federal Court.
