What Is Remote Patient Monitoring (RPM)?

  • Remote Patient Monitoring (RPM) is an efficient healthcare delivery method that uses digital technologies to monitor and analyze vital such as your blood pressure in your own home or wherever you are. RPM technology electronically transmits health information from you in your home to your care team at Long Valley Health Center.

  • Long Valley Health Center uses RPM to collect patient vital signs, including blood pressure, vital signs, weight, heart rate, blood sugar levels, and physical activity. Your data is safe, secure, and will never be sold to a third party.

Who Should Consider RPM?

Our RPM program is currently for patients with hypertension. This group is open to those patients who:

1.       Have high or low blood pressure

2.       Have WiFi or cellular coverage in their home

3.       Are willing to take their blood pressure regularly to stay healthy

How Much Does RPM Cost?

The U.S. federal government reimburses Long Valley for most, if not all, of the expense of RPM. Depending on your health plan, a co-pay of $5 to $20 monthly may apply.
