Community Health Centers

Community health centers such as Long Valley Health Center are community-based and patient-directed organizations that serve populations with limited access to health care. As a community health center, Long Valley Health Center is expected to serve as core safety net providers in meeting the increased demand for care and maintaining delivery of clinical preventive services in North-Central Mendocino County. Long Valley Health Center is a source of primary care for thousands of patients seeking a quality source of care in Northern California. 

As a community health center, Long Valley Health Center identifies the community health needs every two years through a Community Health Needs Assessment. Part of this assessment surveys community members for their thoughts on the local health care needs. This survey helps our Board of Directors understand those needs and develop programs and services to better meet them.

Video Medicine

• Primary care telehealth and telepsychiatry available at LVHC

Ongoing Patient Surveys

• To provide opportunities for patient feedback so we can hear your compliments and concerns
