Long Valley Health Center Awards

HRSA awards Community Health Quality Recognition (CHQR) badges that recognize Health Centers that have made notable quality improvement achievements in the areas of access, quality, health equity, health information technology, and COVID-19 public health emergency response. Long Valley Health Center has received awards every year since the HRSA CHQR program started in 2015.

2023 HRSA CHQR Awards

LVHC was awarded 4 Community Health Quality Recognition (CHQR) awards this year - COV-19 Public Health Champion, Addressing Social Risk Factors, Advancing HIT for Quality and PCMH Recognition. LVHC received these awards because:

  • LVHC is in the top 10% of health centers providing COVID-19 vaccinations or testing to our patients

  • We increased the percentage of patients who received case management services in the last two years

  • We use Health Information Technology to provide quality services such as ePrescribing, Telehealth and a Patient Portal

  • LVHC is a Patient Centered Medical Home

We tied with our sister clinic Anderson Valley Health Center in Boonville for the most (and same) awards in the 15 NCCN-HANC northern California community clinic networks. Congratulations to our award-winning LVHC and AVHC Care Teams!
